Description

1. The subject of the contract is the supply of medical devices for ECMO treatments for the CAU in the assortment and requirements specified in Annex 3 to the SWZ.
2. The Ordering Party requires that the offered assortment constituting a medical device meets the requirements for medical devices contained in the Act of 7 April 2022 on medical devices (Journal of Laws No. Journal of Laws 2022, item 974 as amended) and in implementing acts issued on the basis of this Act.
3. Medical devices must meet the requirements set out in Annex 3 to the CVT. Failure to meet at least one of the requirements set out in Annex 3 to the SWZ will result in the rejection of the tender.
4. Medical devices referred to in para. 1 The Contractor shall be obliged to deliver at his own expense to the Medical Supplies Department, ul. Smoluchowski 17.
5. The delivery of medical devices of this order will be carried out on the basis of partial orders. The maximum delivery time is 6 working days from the date of placing the order by the Ordering Party
