The subject of the contract are successive deliveries of medicinal products used in drug and chemotherapy programs for SPZOZ WSS No. 3 in Rybnik. The tender specifications are set out in Annex 1.1 to these tender specifications (‘Price schedule’). The remaining terms of the contract are set out in Annex 2 to the Specifications, i.e. ‘Draft contract terms’ to be incorporated into the contract/contract terms. Contracts for the awarded packages will be concluded after the end of this procedure with a period of validity until 31.12.2025. The Contracting Authority allows partial tenders, and therefore the subject of the contract is divided into 3 parts (packages). The Ordering Party allows the possibility of submitting tenders for selected parts (packages), but does not allow the possibility of submitting tenders for any selected items in a given part (in a given package), in individual parts (packages) the full range provided by the Ordering Party must be valued - if applicable. The Contracting Authority informs that, in accordance with Article 139 of the Public Procurement Act, it will first examine and evaluate the tenders, and then the qualitative selection of the Contractor whose tender was the highest rated in a given package, as regards the lack of grounds for exclusion and fulfilment of the conditions for participation in the procedure. In complying with the obligation under Art. 6 of the Act of 14 June 2024 on the protection of whistleblowers (Journal of Laws of 2024, item 928), the contracting authority informs that in the Independent Public Health Care Institution of the Provincial Specialist Hospital No. 3 in Rybnik, a procedure for internal notifications of information about violations of law and follow-up actions has been adopted.
