1. The subject of the contract is: Supply of medical devices divided into 6 parts: Part 1 – Model of a newborn for nursing activities 3 set. Part 2 - Model of child care, newborn 3 set. Part 3 – Nursing phantom of a newborn girl 4 pcs. Part 4 – Nursing phantom of a newborn boy 3 pcs. Part 5 – Breastfeeding learning model 2 sets Part 6 – Leopold grip trainer and foetal heart monitor 1 set 2. ANNEXES CONCERNING THE SUBJECT MATTER OF THE CONTRACT: - Description of the subject-matter of the contract - Annex 2 to the FTZ, - Table of conformity of the offered subject-matter of the contract with the DNSH principle (Do No Significant Harm) - Annex 2a to the FTZ, - Table of assessment of warranty conditions - Annex 4 to the FTZ, - Terms of warranty, warranty and warranty service - Annex 5 to the FTZ, - Procedure for delivery and collection of the device - Annex 6 to the FTZ.
