1) The subject of the contract is the provision of expert services in the field of inclusive education. . 2) The main objectives of the project are: a) implementation of organizational changes in public psychological and pedagogical clinics, including specialist ones (hereinafter referred to as: PPP), in the province covered by the subject of the contract; b) complementing the resources for PPP equipment necessary to achieve the above-mentioned objective. . 3) The scope of the contract includes the provision of expert services in the number of: Part A: 360 hours, an average of 10 hours/month (Kuyavian-Pomeranian Voivodeship). b) Part B: 180 hours, an average of 5 hours/month (Lubelskie Voivodship), c) Part C: 180 hours, an average of 5 hours/month (Lubuskie Voivodship), d) Part D: 360 hours, an average of 10 hours/month (Łódzkie Voivodeship), e) Part E: 360 hours, an average of 10 hours/month (Małopolskie voivodship), f) Part F: 180 hours, an average of 5 hours/month (Opolskie Voivodship), g) Part G: 360 hours, an average of 10 hours/month (Pomeranian Voivodeship), h) Part H: 360 hours, an average of 10 hours/month (Silesian Voivodeship), i) Part I: 180 hours, an average of 5 hours/month (Swietokrzyskie Voivodship), j) Part J: 360 hours, an average of 10 hours/month (Warmian-Masurian Voivodeship), k) Part K: 360 hours, an average of 10 hours/month (Wielkopolskie Voivodship), l) Part L: 360 hours, an average of 10 hours/month (West Pomeranian Voivodeship), . 3) As part of the service provided, the Contractor should: a) ensure the participation of (1) the expert (minimum 2 times/month) in on-line meetings (with other team members) within the team of experts created in each of the 12 voivodships (concerns the work of teams within the voivodships/projects for which the Contractor submitted a tender) under the leadership of the head of the expert team to discuss the most important issues related to the implemented change; (b)develop the topics and topics identified in the panel of experts; c) respond on an ongoing basis to emerging issues raised by trainers, supervisors or individual meetings; (d)participate in meetings with specific PPPs to support the change process. . 4) A detailed description of the subject of the contract is attached as Annex 2 to the SWZ; . 5) The Contracting Authority does not provide for the possibility of submitting variant offers. 6) The Contracting Authority does not provide for the award of contracts for similar services within the meaning of Art. 1 point 7 of the Public Procurement Act. . 7) Duration of the contract: from the date of conclusion of the contract until 31 March 2028 or until the number of expert working hours provided for under a given lot has been exhausted, whichever occurs first. . 8)Address of service provision: not applicable (on-line meetings), . 9) The Contracting Authority provides for the payment of remuneration in parts for each calendar month in which expert activities were carried out.
