Powstania Styczniowego 1, - 81 - 348 Gdynia, ul. Wójta Radtkego 1, - 84 - 200 Wejherowo, ul. dr. A. Jagalskiego 10. 2. Szczegółowy opis przedmiotu zamówienia, asortyment i szacunkowe ilości podano w Formularzu Asortymentowo - Cenowym (załącznik nr 2 do SWZ) oraz w Projekcie Umowy (załącznik nr 5 do SWZ).
