I. 1. 1. The subject of the contract is carrying out conservation activities in xerothermic habitats in 9 Natura 2000 areas in the Miechowska Upland. Item has been divided into 2 lots: (1) Part I; Carrying out conservation activities in 4 Natura 2000 sites: Cybowa Góra PLH120049, Grzymałów PLH120053, Wały PLH120017, Giebułtów PLH120051. (2) Part II: Carrying out conservation activities in 5 Natura 2000 sites: Komorów PLH120055, Uniejów Parcele PLH120075, Chodów-Falniów PLH120063, Widnica PLH120076, Pstroszyce PLH120073 3. Contracting authority – pursuant to Art. 1 of the Act – requires the Contractor or subcontractor to employ, on the basis of the employment relationship, persons referred by the Contractor or subcontractor to perform activities involving physical work, such as: mowing, cutting, ringing, grubbing up, collecting biomass, if the performance of these activities consists in performing work in the manner specified in Article 22(1) of the Labour Code of 26 June 1974 (Journal of Laws 1974, No. Journal of Laws 2023, item 1465, 2024, item 878, 1222). 5. The Contracting Authority allows the submission of partial tenders as part of the division of the contract into 2 lots, indicated in point. 1 above. Documents to be submitted by the contractor together with the tender: 1) the completed Offer Form (Annex 1 to the SWZ); 2) 3) a power of attorney or other document confirming the power of attorney to represent the contractor – if the tender or the statements or documents attached to it are signed by a proxy. 4) power of attorney to represent all Contractors jointly applying for a contract, possibly a cooperation agreement, from which the power of attorney will result. 5) EAT (in accordance with the instructions in Annex 2 to the FTZ), appropriate as at the date of submission of tenders, in order to temporarily confirm that the Contractor is not subject to exclusion from the procedure pursuant to Art. 1 and Article 109 para. 1 point 4 of the PPL Act and meets the conditions for participation in the procedure set out in the FTZ. Where the Contractors jointly apply for a contract, the SEDZ shall be submitted by each of the Contractors jointly applying for the contract; 6) SEDZ concerning other entities referred to by the Contractor, in order to temporarily confirm, as at the date of submission of tenders, that there are no grounds for exclusion in respect of them pursuant to Art. 1 and Article 109 para. 1 point 4 of the Public Procurement Act and meeting the conditions for participation in the procedure specified in the FTZ, to the extent that the Contractor refers to their resources; 7) where the Contractor relies on the capacities or circumstances of the entities making resources available: evidence that the Contractor will have at his disposal the necessary resources of other entities when performing the contract, which constitutes, in particular, an obligation on those entities to place at the Contractor’s disposal the necessary resources for the performance of the contract or other subjective means of proof that the Contractor will have at his disposal the necessary resources of those entities when performing the contracts.
