Description

The subject of the contract is the supply of medical materials for cataract surgery using the phacoemulsification method together with the lease of a phacoemulsifier for the Ophthalmology Department. The contracting authority did not divide the contract into lots and therefore does not allow partial tenders to be submitted. Reasons for not dividing the contract into lots. A detailed description of the subject of the contract is contained in Annex 1A to the SWZ, which is an integral part of it. The Contractor is obliged to successively deliver medical materials within 3 working days from the date of placing the order. The Contractor (the lessor) is obliged to deliver the leased device with the equipment, as well as to install it, commission it at his own cost and risk at the Ophthalmological Department of the Health Care Establishment Complex in Ostrów Wielkopolski at ul. Limanowskiego 20/22 and to train the staff at the Lessee's headquarters in the operation and operation of the device within 7 working days of the date of signing the contract, after prior agreement with the Head of the Ophthalmology Department. For the duration of the contract, the Contractor undertakes to provide the Ordering Party free of charge with a set of lenses and a reusable implant kit in the quantity and with the parameters specified in Annex 1A to the SWZ within 7 working days from the signing of the contract.
