Description

1. The contract concerns:
a. Supply of reagents for serological testing in the range and the estimated quantity set out in Annex 6 to the ToR.
b. Less of two serological analysers in accordance with Annex 7 to the ToR.
2. The contracting authority would point out that the quantities of the range covered by the contract set out in Annex 6 to the ToR are estimates used to correctly calculate the price of the tender, to compare the tenders and to select the most advantageous tender. In the performance of the contract, the quantity of the product ranges ordered may be reduced or increased to 25 % per product group (in words: twenty-five per cent), depending on the Contracting Authority’s actual needs, provided that the value of the successful Contractor’s tender is not exceeded.
3. The supply of reagents for serological tests will be carried out on the basis of partial contracts. Max delivery date: 5 working days from the date of placing of the order. The Contractor will be required to provide the requested range at his own expense.
4. Listed in point. 1 reagents shall meet the requirements of Annex 6 to the ToR. Failure to comply with one of the requirements set out in Annex 6 to the ToR will lead to rejection of the tender.
5. The expiry date of the reagents at least 6 months from the delivery date.
6. Each reagent must have a specified expiry date and a leaflet in Polish.
7. Listed in point. 1 b analysers must meet the parameters set out in Annex 7 to the ToR. Failure to comply with one of the limit parameters set out in Annex 7 to the ToR will lead to the rejection of the tender.
8. In addition, the Contractor will be responsible for:
— the installation of the supplied analysers at the direct user;
— the provision of maintenance training on the rational operation of the analysers provided;
— the software update of the apparatus during the duration of the contract;
— provision of instructions for use in Polish: printed and electronically on the CD (to be delivered with the delivery of the leased item);
—provision of servicing services for analysers rented by the Lessor, including rent rent surveys, maintenance, repairs and software updates;
— the possibility to report accidents 24 hours a day within 365/366 days a year;
— the provision of 48 monthly guarantees;
— remedying the failure or issuing of a replacement device by the Lessor Service within a period of not more than 48 hours after the contracting authority has notified the damage.
9. The Contractor shall be required to insure, at its own expense, the leased object against all risks relating to: theft, fire, flooding, explosion, destruction, loss of leased object and to maintain such insurance throughout the duration of the contract.
10. The contractor will provide a guarantee in accordance with the tables of technical parameters set out in Annex 7 to the ToR calculated from the date of the receipt of the equipment by the Contracting Authority. The guarantee will cover all equipment supplied and installed by the Contractor.
11. The contracting authority shall not allow the submission of partial tenders.
12. The contracting authority shall not allow variants to be submitted.
