Description

The subject of the assignment includes the production of spectacles for the correction of vision (diopter) of the workers to whom they are prescribed, after examination by the ophthalmologist who carried out a preventive examination of the employee in 2017-2016, under a contract with the Medical Centre ‘Lora’ and after consultation with the contractor’s specialists to achieve optimal correction and comfort. Eye correction glasses shall be made only with the consent of the entitled person, an individual order signed by him or her and the presentation of an original recipe. The lists of persons entitled by structural units at the Customs Agency shall be transmitted to the contractor by the person designated by the contract with a cover letter after signature of the contract. If necessary, the person designated in the contract may amend or supplement the lists submitted to the Contractor and shall inform the Contractor thereof.
