Description

Przedmiotem zamówienia jest: Dostawa sprzętu do zabiegów hemodynamicznych.
Zamawiający podzielił zamówienie na następujące zadania:
Zadanie nr 1 – Mikrocewniki
Zadanie nr 2 – Prowadniki ciśnieniowe, sondy do OCT + dzierżawa
Zadanie nr 3 – Cewniki mechaniczne do ultrasonografii wewnątrzwieńcowej + dzierżawa
Zadanie nr 4 – Cewniki z balonem monorail do zabiegów PCI w zmianach złożonych i krętych naczyniach
Zadanie nr 5 – Cewniki do pomiaru ciśnienia zaklinowania
Zadanie nr 6 – Introducery transradialne hydrofilne
Formularz asortymentowo-cenowy w podziale na poszczególne zadania stanowi Załącznik nr 1a do niniejszej SWZ.
Opis przedmiotu zamówienia w podziale na poszczególne zadania stanowi Załącznik nr 2 do SWZ.
Szczegółowe warunki realizacji i rozliczenia zamówienia określa Załącznik nr 8 i 9 do SWZ –Projektowane postanowienia umowy.
