Description

1. The subject of the contract is the supply of medical devices, including disposable surgical instruments and dressing kits, hereinafter referred to as devices. 2. The description of the subject-matter of the contract and the estimated quantities relating to the products are set out in the unit price form (Annex I). 2 to the SWZ, hereinafter also referred to as FCJ). 3. The contracting authority requires the offer of medical devices within the meaning of the Medical Devices Act of 20 May 2010 (Journal of Laws 2020, item 186, as amended), hereinafter: UoWM), placed on the market and used on the territory of the Republic of Poland in accordance with the applicable regulations. 4. The Ordering Party requires that the shelf life of all offered products is a minimum of 12 months, counting from the date of delivery to the Ordering Party's registered office. 5. The Ordering Party has the right to order a smaller quantity of products, with the exception that not less than 60% of the quantity specified in the unit price form. 6.
