Description

The scope of the contract covers XVI lots (packages) – Annex 1.
Package I.
Purchase of agarose gel electophoresis kits adapted to Cormay’s DS2 apparatus and densitometer under the maximum quantities set out in Annex 1.
Package II.
Purchase of micros 60 haematological analyser reagents in the range of maximum quantities specified in Annex 1.
Package III.
Purchase of latex tests, cassette tests and general analytics reagents in the range, limit conditions, packaging, maximum quantities set out in Annex 1.
Package IV.
Purchase of disposable laboratory equipment and glassware in the combination, limit conditions, packaging, maximum quantities set out in Annex 1.
Package V.
Biological indicators in the range, maximum quantities set out in Annex 1.
Package VI.
Sheep blood in the composition, packaging, the maximum quantities set out in Annex 1.
Package VII.
Microbial substrates in the composition, packaging, maximum quantities set out in Annex 1.
Package VIII.
De-sensitivity (substrate, antibiogram discs) in the range, packages, the maximum quantities set out in Annex 1.
Package IX.
Latexs, diagnostic discs in the assortment, packaging, maximum quantities set out in Annex 1.

Additional tests in the composition, packaging of the maximum quantities set out in Annex 1.
Package XI.
Immunochromatographic tests, identification, medio-sensitivity in the combination, packaging, maximum quantities set out in Annex 1.
Package XII.
Gram staining in the composition, packaging, of the maximum quantities set out in Annex 1.
Package XIII.
Standard strains in the range, packages, the maximum quantities set out in Annex 1.
Package XIV.
Serums, latex in the assortment, packings, the maximum quantities set out in Annex 1.
Package XV.
Laboratory glass and small equipment in the range, packaging, the maximum quantities set out in Annex 1.
Package XVI.
Articles of plastics or other materials in the composition, packaging, the maximum quantities set out in Annex 1.
Detailed requirements relating to the performance of the subject matter of the contract for package II.
1. All reagents, calibrators and controls must come from one manufacturer.
2. All reagents offered must bear the CE marking.
3. The contractor providing the reagents specified in package II shall provide, free of charge, a service review of Micros 60 analyser, together with the replacement of the consumable parts of the analyser identified under CPV code 38434570-2.
4. Control blood must control all parameters determined on the analyser.
Detailed requirements for the performance of the subject-matter of the contract for packages from V-XVI:
Together with the tender, the contractor will provide in electronic form (excel file or similar) the tender form in the order given in the Contracting Authority’s tender form.
Specific requirements for performance of the subject-matter of the contract:
1. Certificates, instructions, safety data sheets must be attached to the tender for each product.
2. Certificates in an electronic version on the Contractor’s website.
3. ISO 9001 certificate for the manufacture of finished substrates on wafers and test tubes, antibiotic discs, latex tests.
4. Quality control certificates for each series of substrates on plates and tubes, which shall contain:
— name of the manufacturer, name of the product, batch number, expiry date,
— composition of the medium,
— safety data sheet,
— general characteristics of the medium: colour, pH, sterility, packaging,
— microbiological characteristics, quantification of fertility and selectivity, together with a description of the morphology of colonies developed on the medium,
— for Mueller medium Hinton, the stability control of the medium with individual strains of ATTCC and antibiotic discs.
5. Parameters for finished substrates on tiles and tubes:
— 9 cm plate diameter,
— the printing on the plate should include the name of the medium, the serial number, the expiry date,
— term of validity of the substrates on tiles: minimum 4-6 weeks for blood substrates, minimum 6-8 weeks for other substrates.
6. Mueller substrate – Hinton and antibiotic discs mentioned in the package
Susceptibility must come from one manufacturer. Expiry date of antibiotic discs at least 1 year.
7. Each disc vial should be packed in a separate, hermetic sealed moisture absorber packaging.
8. The symbol and concentration of μg must appear on each individual disc, marking on both sides of the disc.
9. The chromogenic substrates on the plates must be accompanied by a coloured leaflet in Polish.
10. In the case of latex ummunochromatographic and other tests, the Contractor should attach the performance procedure – an instruction in Polish.
11. Subject: Package VII: capacity refrigeration cabinet. 250-300 l, glass doors, dimensions max: 80x60x200 cm. Payment divided into 24 instalments.
12. Subject to package VIII: thermometer calibrated, non-mercury graduated 0.5 °C, min. Temperature range:—70 to + 70 C.
13. Subject: Package V biological indicators shall be adapted to the DRY BLOCK Incubator 60 C – SPS medical incubator.
14. Subject: Package XIII: standard strains – lyophilised in discs.
15. The Contractor undertakes to consult on the merits.
16. If quantitative or qualitative deficiencies are found, the Contracting Authority shall immediately notify the contractor, which will examine the complaint concerning quantitative and qualitative deficiencies within 48 hours. The Contractor undertakes to replace the defective good without delay for a good free of defects and to make good any quantity defects in the goods delivered within 48 hours of the examination of the complaint or after the expiry of the time limit for examining the complaint.
