Description

1. The subject of the contract is the supply of tests and sets of PCR II round for the Veterinary Inspectorate of the Provincial Veterinary Inspectorate in Szczecin, to the extent specified in Annex No 1 to the FTZ, broken down into 12 lots. 2. The detailed scope and requirements for each part of the procedure are set out in Annex 1 to the FTZ, which is an Assortment and Price Form. 3. The detailed manner of performance of the subject-matter of the contract is set out in Annex 2 to the FTZ constituting the draft provisions of the contract. 4. PROVISIONAL PROVISIONAL MEASURES: 1) The Ordering Party informs that in each part of the procedure, in order to confirm the conformity of the offered supplies with the requirements, characteristics or criteria specified in the description of the subject of the contract, the Ordering Party requires that the Contractor, in the case of offering equivalent products, submit with the offer: a) reports of initial validation of the offered equivalent products, in Polish or translated into Polish, specifying, among others, the name of the manufacturer, catalogue number, product parameters, indicating which part of the procedure and which item within this part these documents relate to; b) the quality specification, quality control certificate or other document, which clearly shows that the equivalent product offered meets the requirements of the description of the subject of the contract, in Polish or translated into Polish, specifying, among others, the name of the manufacturer, catalogue number, product parameters, indicating which part of the procedure and which item within this part these documents relate to. 2) The Ordering Party may request explanations regarding the content of the evidence in question. 5. All other requirements concerning the means of proof and equivalence in question, which are not included in the notice in question, are contained in the FTZ and the annexes to the FTZ.
