The subject-matter of this call for tenders is the supply of medicinal products, including within the framework of drug programmes and medical devices, hereinafter referred to as ‘goods’, in accordance with the type of assortment and the quantities set out in the ‘Assortment and Pricing Form’ in Annex 2 to the CVT and in accordance with the ‘Offer Form’ in Annex 1 to the CVT. The order is divided into 58 packages. The Assortment and Price Form, in the relevant columns of the individual packages, indicates the quantities that will be ordered by the Independent Public Department of Health Care, University Clinical Hospital No. 2 of the Medical University of Lodz, the Independent Public Department of Health Care, University Clinical Hospital No. 1 of the Medical University of Lodz and the Independent Public Department of Health Care, Central Clinical Hospital of the Medical University of Lodz.
