1. The subject of the contract is: PURCHASE OF MEDICAL APPARATUS FOR THE OPERATIONAL TREATY implemented as part of the special purpose grant received for the investment project under the name: ‘Increase in standards of medical and technical infrastructure in the area of the Operational Route and the Birth Block with the Caesarean Section Room through comprehensive modernisation of the premises and full energy and fire protection of buildings B and C of the Provincial Specialist Hospital named after the Blessed Virgin Mary in Częstochowa at ul. Bialska 104/118’. 2. The detailed scope of the contract is set out in Annex 2 to the SWZ – assortment and price form and in Annex 3 to the SWZ – description of the subject-matter of the contract. Other conditions relating to the performance of the contract are set out in the draft provisions of the contract, which are Annex 7 to the SWZ. 3. The subject of the contract consists of 9 lots. The contracting authority shall allow partial tenders to be submitted. The term ‘partial tender’ shall mean the individual lots set out in Annex 2 to the SWZ. The contractor may submit a tender for any of the lots or for the entire subject-matter of the contract: PART 1 — Mobile ultrasound scanner — 2 units PART 2 — Operating table — 2 pcs PART 3 – Surgical toolkit – 1 set PART N° 4 — Infusion pump — 4 pcs PART 5 — TIVA infusion pump — 4 pcs PART 6 — Docking station — 2 PART No 7 — Apparatus for heating infusion fluids — 2 pcs PART 8 — Patient heating appliance — 1 pcs PART N° 9 — Anaesthesia trolley — 2 pcs 4.
