Description

1) The subject of the contract is the provision of an expert service in the field of cross-sectoral cooperation in the Warmian-Masurian Voivodeship. The main objectives of the project are: a) implementation of organizational changes in public psychological and pedagogical clinics, including specialist ones (hereinafter referred to as: PPP), in the voivodship covered by the contract, b) supplementing the resources in the field of PPP equipment necessary to achieve the above-mentioned objective. 2) The scope of the contract includes the provision of expert services in the number of: 360 hours, an average of 10 hours/month. As part of the service provided, the Contractor must: a) ensure the participation of (1) an expert (minimum 2 times/month) in on-line meetings (with other team members) within a team of experts established in a given province under the leadership of an expert team leader to discuss the most important issues related to the implemented change, b) develop the issues and topics established in the team of experts, c) respond on an ongoing basis to emerging issues reported by trainers, supervisors or individual meetings, d) participate in meetings with specific PPPs in order to support the process of the introduced change. 3) A detailed description of the subject of the contract is attached as Annex 2 to the FTZ; 4) The Ordering Party does not provide for the possibility of submitting variant offers. 5) The Contracting Authority does not provide for the award of contracts for similar services within the meaning of Art. 1 point 7 of the PPL Act. 6) Order completion date: from the date of conclusion of the contract until 31.03.2028 or until the number of hours of the expert's work is exhausted, whichever occurs first. 7) Address of service provision: not applicable (online meeting), 8) The contracting authority provides for the payment of remuneration in parts for each calendar month in which the expert activities were carried out. Remuneration shall be payable within 30 days from the date of receipt by the Ordering Party of the VAT bill/invoice for the provision of the service. 9) Detailed conditions for the performance of the contract and terms of payment are included in the model contract constituting Annex 3 to the FTZ. 10) The Contracting Authority does not provide for additional requirements related to the performance of the contract concerning social, economic, environmental aspects, related to innovation, employment or maintaining the confidential nature of information provided to the contractor during the performance of the contract; 11) The Ordering Party does not provide for a reserved clause within the meaning of Article 94 of the PPL; 12) Due to the type of contract, the Employer does not provide for an employment obligation for an employment contract referred to in Article 95 of the Public Procurement Act. 13) The Contracting Authority does not provide for an obligation for individual contractors jointly applying for a contract within the meaning of Article 60 of the Public Procurement Act to perform key tasks concerning service contracts in person, or related to the participation of entities providing resources within the meaning of Article 121 of the Public Procurement Act. 14) The Contractor may subcontract part of the contract. The Ordering Party does not reserve the obligation for the Contractor to personally perform the key parts of the contract; The contractor must indicate in the tender the parts of the contract which it intends to subcontract and, if known, the names (companies) of those subcontractors. 15) The description of the criteria for the evaluation of tenders, including the weightings of the criteria and the method of evaluation of tenders, is described in detail in chapter. XIII FTZ.
