1. The subject of the contract is delivery to the Provincial Hospital in Bielsko-Biała at Al. Armii Krajowej 101 implants and endovascular equipment for neuroradiological and neurosurgical procedures in accordance with the quantitative and generic list set out in Annex No 2 to the Specifications of the Contract Terms and Conditions, hereinafter referred to as ‘the swz’, including by depositing with the Ordering Party for Parts 1-5, meeting the quality parameters set out in Annex No 2 to the swz, under the conditions specified in the draft provisions of the contract constituting for Parts 1-5 – Annex No 4a to the swz, for Part 6 – Annex No 4b to the swz. 2. The offered goods must be new and unused. 3. Delivery of the subject of the contract together with transport and unloading will take place at the expense and risk of the Contractor. 4. Applicable to Section 5 – For the duration of the contract, the Contractor is obliged, as part of the price of the subject matter of the contract, to hand over and deposit at the premises of the Contracting Authority, two sets of tools composed of: a cross screwdriver, ticks for cutting plates, ticks for bending plates, tweezers, a system for storing a magazine for 100 screws, magazines for 6 screws, space for plates and a screwdriver blade, a mesh drawer, for implantation of implants listed in Part 5, together with a container for sterilization and sterile storage of instruments.
