Description

1. The subject of the contract is the supply of medical equipment specified in parts for the Fryderyk Chopin University Clinical Hospital in Rzeszów 2. A detailed description of the subject matter of the contract, indicative quantities of supplies and requirements were included in the Description of the subject matter of the contract – a list of parameters required for Parts Nos 2,3,5 and 7 and a list of parameters required and assessed for Parts Nos 1, 4, 6,8 – Attachments Nos 1 to the SWZ and Price and Offer Forms for Parts Nos 1 to 8 – Attachments Nos 2 to the SWZ, which are an integral part of the SWZ. 3. The scope of the contract includes the delivery of the items of the contract in quantities of: - X-ray apparatus with a dip – 1 pc, - Ultrasound apparatus – type I – 1 pc, - Ultrasound apparatus – type II – 1 pc, - Ultrasound apparatus – type III – 2 pc, - Endoscope set (Colonoscope – 1 pc, Duodenoscope – 1 pc, Gastroscope – 1 pc, Ultrasound gastroscope – 2 pc) – 1 set, - Vital function monitoring system – 1 set, - Microsurgical drive – 1 set, - X-ray apparatus – 1 pc, described in Annexes 1 and 2 to the CVT. 4. The Contracting Authority allows submission of partial tenders, where each complete part is a part. Tenders may be submitted for one, several or all lots. There are 8 parts to this process. 5. The Contracting Authority shall not allow the submission of tenders: (a) variants. (b) equivalent in Parts 2, 3 and 5. 6. In Lots 1, 4, 6, 7, 8, the Contracting Authority allows solutions equivalent to those described in the description of the subject of the contract: Whenever the description of the subject-matter of the contract refers to standards, European technical assessments, approvals, technical specifications, technical reference systems referred to in Art. 1 point 2 and sec. 3 PPL, it is assumed that such indications are accompanied by the words "or equivalent". Note applies to Parts 1 and 8. The Contracting Authority informs that before submitting the tender, due to the specific nature of the subject of the contract, it requires the Contractor to conduct an on-site visit to the Contracting Authority in order to learn about the location of the contract and the technical condition of the premises (X-ray laboratories at the Clinical Department of Radiology and Imaging Diagnostics), where adaptation works of the laboratory infrastructure are planned to the extent necessary to install and launch the offered X-ray machine with a copy – Part No. 1 and X-ray machine – Part No. 8, as well as to check in detail in the field the conditions for the performance of the contract and possible difficulties that may occur during their implementation. The Contractor shall carry out the on-site inspection at his own expense. In order to arrange a local visit, the Contractor should contact the Medical Equipment Department of the Ordering Party, tel. 17 8666 050. The visit will take place on the date set by the Ordering Party until 10.01.2025. Arriving persons should carry an ID card and comply with the applicable sanitary regime. A report will be drawn up on the vision carried out – the model is set out in Annex 11 to the CVT – and concerns Parts 1 and 8 . The contracting authority informs that in accordance with Art. 1 point 18) The Contracting Authority will reject the tender submitted in Parts 1 and 8 if it was submitted without an on-site visit. 8. DEADLINE FOR COMPLETION OF THE CONTRACT: 1) Delivery of the subject of the contract in Part No. 1-8 to the place indicated by the Ordering Party by 30 April 2025. 2) Commissioning of the delivered equipment, staff training and commissioning into clinical operation in Part No. 2-7 by 31 May 2025, confirmed by the signing of the final protocol. 3) The commissioning of the delivered equipment in parts 1 and 8 by 31 May 2025, confirmed by the signing of the final protocol. 4) In parts no. 1 and no. 8 (delivery of x-ray equipment with a kick-off and x-ray equipment) - the date of commissioning and training of personnel by 31 July 2025, after agreeing with the Ordering Party the date of obtaining a permit for the use of new x-ray equipment from the Provincial Sanitary and Epidemiological Station in Rzeszów. 9.The Contracting Authority shall conduct a public procurement procedure for the purchase of the objects of the contract based on the grant agreement No PLUA.02.01-IP.01-0108/23-00 for the implementation of the project: Common challenges in the treatment of patients using laparoscopic techniques in urology, oncology and emergency medicine in accordance with the decision of the Monitoring Committee of 15.12.2023, as part of the Interreg NEXT Poland-Ukraine 2021-2027 Programme concluded between the Ministry of Development Funds and Regional Policy of the Republic of Poland and the Contracting Authority.
