1. The subject of the contract is the supply of medical materials used in gynecological surgery for the needs of the Międzyleski Specialist Hospital in Warsaw. 2. The subject of the contract was divided into 14 task forces, in the quantities and assortment indicated in the Price Forms constituting integral parts of this SWZ. 3. The offer may apply to all or selected Tasks. Submission of partial tenders within individual Tasks is not allowed. 4. The contracting authority shall not allow variants to be submitted. 5. Each Contractor shall be entitled to submit only one tender. The submission by one Contractor of more than one tender or of a tender containing alternatives shall result in the rejection of all tenders submitted by that Contractor. 6. The assortment offered in the tender procedure (listed in the Price Forms) must comply with the description of the subject of the contract and must meet the conditions for admission to trading in health care institutions in accordance with the applicable law. 7. Failure to meet any of the required parameters gives the Ordering Party a basis for rejecting the offer. 8. The contract will be performed for a period of 12 months from the date of conclusion of a valid contract with the Contractor selected as a result of the public procurement procedure. 9.Delivery time can not be more than 3 days. 10.Tenders will be evaluated on the basis of the criteria described in Chapter XV of this SWZ. 11. The draft provisions of the contract constitute Appendix No. 1 to the SWZ, which is an integral part of this documentation.
