Description

1. The subject of the contract is: Comprehensive nutrition service for patients of the Ludwik Rydygier Provincial Hospital in Suwałki, together with the lease of a kitchen and the running of a buffet. 2. A detailed description of the subject-matter of the contract is contained in Annexes 4, 5, 6 and 7 to the SWZ and in the model contract constituting Annexes 2 and 2a to the SWZ. 3. The service provided includes: a) patient nutrition service consisting in the preparation of meals on the basis of leased kitchen rooms with equipment, b) running a buffet for the needs of customers / employees, patients and other persons / on the basis of leased rooms, devices and equipment of the hospital buffet, c) replacement with the installation of a belt tunnel dishwasher, together with an automatic water softener, replacement of the tray system with equipment, replacement of trolleys for transporting thermal insulation trays with plate distributors in accordance with Annex 4 to the Terms of Reference (d) renovation of the main kitchen rooms, employee rooms and the central washing room with the implementation of a new comprehensive horizontal insulation and the location of a new floor in the main kitchen and the washing room, in accordance with Annex 6 to the Terms of Reference The Contractor is obliged only to carry out part of the construction work, while the work of the electrical and sanitary industries and other construction works will be carried out by the Contracting Authority. 4. The subject of the contract must be performed in accordance with the material and financial schedule, hereinafter referred to as the schedule, developed by the Contractor and accepted by the Ordering Party. 5. The contractor will prepare a schedule within 7 working days and agree it with the contracting authority before concluding the contract for this public contract. Supervision of the works will be carried out by a designated employee of the Employer. 6. The Contracting Authority shall make available to the Contractor the premises of the General Kitchen and the Buffet, together with the equipment and other equipment listed in Annex 7 to the TMS, on the terms agreed by a separate lease agreement for the premises of equipment and equipment, on the basis of a delivery and acceptance report drawn up and signed by the persons designated by the Parties to the contract. 7. The Contractor is obliged to provide the service in accordance with the applicable provisions of law, in particular the Act of 25 August 2006 on food and nutrition safety (Journal of Laws No. Journal of Laws 2023, item 1448.t.j.) 8. All elements of the service provided must comply with the procedures of the HACCP system, the principles of GHP Good Hygiene Practice, the principles of GMP Good Manufacturing Practice. 9. The Contractor will start the renovation works within 6 months from the date of signing the contract.
