Description

1. The subject of the order are successive deliveries on the order of the Ordering Party of medical devices. A detailed description of the subject-matter of the contract is provided in Annex 2 to the FTZ – Assortment and price form The subject-matter of the contract is divided into 20 individual tasks: Task 1. Assortment of anaesthesia Task 2. Drains for suction Task 3. Respiratory systems and filters Task 4. Resonance syringes Task 5. Anaesthesiological assortment for stoma and urostomy Task 6. High Flow Therapy Accessories for Humid-BH Task 7. Set for drainage of bile ducts Task 8. Infusion set Task 9. Inserts for mammals Task 10. Disposable nebulisers Task 11. Disposable devices Task 12. Other sterile medical devices Task 13. Neutral electrode and monopolar knife Task 14. Faucet ramps - unconnected faucets Task 15. Sequential compression cuff Task 16. Thigh Sequential Compression Cuff / COVIDIEN Kendall SCD700 SERIES Task 17. Assortment for High Flow Oxygen Therapy Device Manufacturer: Fisher & Paykel Healthcare SAS Model: AIRVO 2 Humidifier Task 18. Assortment of disposable pens MEDTRON - Accutron, owned by the hospital Task 19. Assortment of anaesthesia Task 20. Cartridges for a tomograph - A detailed description of the subject-matter of the contract is provided in Annex 2 to the SWZ Form assortment and price. 2. The medical assortment that is the subject of the contract must be allowed to be placed on the market and used, meet the requirements set out in Regulation (EU) 2017/745 of the European Parliament and of the Council of 5 April 2017 on medical devices (MDR) and in the Medical Devices Act of 7 April 2022 (Journal of Laws 2024, item 1620), in particular bear the CE marking and the corresponding declarations of conformity. 3. The Contractor is obliged to have and, at the request of the Ordering Party within 3 working days, submit all documents confirming the admission of the offered assortment to the market and use, in accordance with the applicable law. The Contractor shall ensure that these documents are kept up to date throughout the duration of the contract.
