Description

Supply of magnetic resonance with anaesthesia apparatus, together with the installation, installation and adaptation of premises for the Immaging Diagnostic and Procedure Radiology Unit and the financing service.
The subject matter of the contract includes:
1) drawing up project documentation, including construction projects and implementing projects in the construction, sanitary and electrical sectors, in accordance with the Regulation of the Minister for Infrastructure of 2.9.2004 on the detailed scope and form of design documentation, technical specifications for the execution and acceptance of construction works and the functional and utility programme (Journal of Laws 2013, item 1129,) on the basis of the functional utility programme annexed to the ToR, together with the necessary arrangements and opinions required by law;
(2) construction works consisting of the reconstruction of magnetic resonance rooms with an anaesthesia apparatus in Blok C (low ground floor) at Stefan Kardynał Wyszyński Specialist Hospital in Lublin, al. Kraśnicka 100 according to the design documentation drawn up, in accordance with the provisions of the building law, the applicable legislation, Polish standards transposing European standards or those of other Member States of the European Economic Area, visit the technical and construction works and the conditions laid down in the contract, together with the supply of the additional equipment indicated in Annex 12 to the ToR;
(3) supply of factory new magnetic resonance together with an anaesthesia apparatus and equipment (including a descriptive station computer) and software,
4) assembling and installation of magnetic resonance with anaesthesia apparatus, together with equipment and software, obtaining approval and final authorisations related to radiation protection and ionising radiation and building regulations (if required by the applicable regulations;
5) perform acceptance tests after installing magnetic resonance with an anaesthesia apparatus and providing the documents necessary for the release of the device into service and obtaining all the permits required by law,
6) activation of installed magnetic resonance with an anaesthesia apparatus and training of staff,
7) a financing service consisting of the payment of remuneration for the performance of the subject-matter of the contract within the scope indicated in points 1-2 in 36 (in words: thirty-six) equal monthly parts.
