Description

1. The subject of this contract is the purchase of equipment for medical rehabilitation for oncological patients of the Provincial Specialist Hospital named after the Blessed Virgin Mary in Częstochowa under the multiannual programme entitled The National Oncological Strategy in the field of the task entitled ‘Purchase of medical rehabilitation equipment for cancer patients’ in 2025, co-financed by the Ministry of Health. 2. The detailed scope of the contract is set out in Annex No 2 to the FTZ – product and price form and in Annex No 3 to the FTZ – description of the subject-matter of the contract. The other conditions relating to the performance of the contract are set out in the draft provisions of the contract, which constitute Annex No 7 to the FTZ. 3. The subject-matter of the contract comprises 9 lots. The contracting authority shall allow partial tenders to be submitted. The term "partial tender" means the individual lots set out in Annex 2 to the FTZ. The Contractor may submit a tender for any lot or for the whole subject of the contract: 1) PART 1 – Diagnostic and therapeutic device for the rehabilitation of the upper limbs – 1 2) PART 2 – Device for the automated rehabilitation of the hands and fingers of the upper limb – 1 pcs. 3) PART 3 – Kinesitherapy device – 1 piece 4) PART 4 – Radiofrequency equipment – 1 piece 5) PART 5 – Deep oscillation apparatus – 1 piece 6) PART 6 – Kinesitherapy device – 1 piece 7) PART 7 – Diagnostic and therapeutic device for the rehabilitation of the upper limbs – 1 8) PART 8 – Device for the automated rehabilitation of the hands and fingers of the upper limb – 1 pcs. 9) PART 9 – Diagnostic and therapeutic device for the rehabilitation of the upper limbs – 1 4. The contracting authority informs about the possibility of cancelling the procurement procedure on the basis of Article 257 of the Public Procurement Act, according to which the contracting authority may cancel the procurement procedure if the public funds that the contracting authority intended to allocate to finance all or part of the contract were not granted to it.
