The subject of the contract is the implementation of the investment ‘Expansion of the Hospital Complex with the execution of the necessary investment tasks for the Hospital of Our Lady of Perpetual Help in Wołomin. Part II" covering the following ranges/options of the Hospital of Our Lady of Perpetual Help in Wołomin (05-200) Wołomin, ul. Gdynia 1/3 (hereinafter referred to as the Ordering Party) along with the reconstruction of land development elements within the limits of the development, taking into account the technical infrastructure necessary for its functioning and the supply of medical and non-medical equipment. The basic scope includes: a) execution of construction works of building A and part of the main building C, so as to adapt the functional layout to the current needs of the Ordering Party. As part of the designed function in the facility (in the part covered by the scope of the study) there will be:  Department of Pathomorphology,  Central Sterilization Plant,  Hospital Emergency Department (SOR),  Entrance zone of the main building C with a waiting room,  Department of Imaging Diagnostics,  Operational Block,  Intensive Care Unit (OIT),  Technical, storage and cloakroom facilities for building A and main building C,  Core of vertical communication (with cranes) between building A and main building C with its incorporation into the main building C and reconstruction of existing rooms adjacent to the floors from -1 to +6. b) execution of construction works in the scope of the newly designed, free-standing concierge building. c) execution of all construction works apart from the above-mentioned buildings necessary for their proper functioning, among others:  major renovation or demolition and new construction of a water treatment plant with a comprehensive replacement of equipment  renovation of the central hydrophore with a comprehensive replacement of equipment  construction / reconstruction of the technical infrastructure on the premises of the Hospital to ensure the proper functioning of medical buildings – construction of a new oxygen line (Ø42) from the existing oxygen plant to the main building C, from which building A is supplied with oxygen. d) supply* of medical and non-medical equipment in accordance with the Annex to the OPZ. *- (each time) delivery, assembly, commissioning, receipt, training of the User.
