Description

1. The subject-matter of the contract is the purchase and successive delivery of chemotherapy medicinal products listed in 31 lots for the Fryderyk Chopin University Clinical Hospital in Rzeszów. 2. Detailed description of the subject of the contract, the Contracting Authority is specified in the Price and Tender Forms – Attachments No. 1 for individual parts, which are an integral part of the SWZ. 3.The Ordering Party requires to offer items of the order which: - meet the requirements described in the Price and Offer Forms, - meet the requirements described in the CVT, - are covered by the Act of 12 May 2011 on the refund of medicines, foodstuffs intended for particular nutritional uses and medical devices – concerns medicinal products covered by the refund. 4. The scope of the contract includes the delivery of the items of the contract in the quantities specified in the Price and Offer Forms for the period of: - Part No 1 to No 22 – from 28 May 2025 for 12 months. - Part No 23 to No 28 – from 19.06.2025 for 12 months. - Lot No 29 – 31 – for 12 months from the date of conclusion of the contract. 5. The Ordering Party in Part No. 1 to No. 31 allows for the possibility of changing the term of the contract concluded with the Contractor in the event of failure to perform the entire subject of the contract during its term. In this case, the amendment of the contract requires the signing of an annex, except that the Contracting Authority shall unilaterally notify the Contractor of the proposed extension of the contract before the expiry of the period for which it was concluded. 6.
