1. Scope of the contract: The subject of the contract is: property insurance against all risks (ALL RISKS); insurance of electronic equipment against all risks; accident insurance of machinery and equipment (MB); − insurance of construction machinery; civil liability insurance for business activity, use of property and placing the product on the market; - third party liability insurance of the entrepreneur maintaining accounting books; civil liability insurance for adverse effects on the environment; −compulsory third party liability insurance for motor vehicle owners; insurance of the consequences of accidents of the driver and passengers of a motor vehicle; Autocasco insurance; ASSISTANCE insurance. 2. The description of the subject of the contract includes the following annexes: − Insurance Scheme, Annex 1 (a); − Property damage report appendix 1 (b) 1; − Report on third-party liability insurance, Annex 1 (b) 2; − Motor insurance report appendix 1 (b) 3; − Integrated Management System Certificate QUALITY* ENVIRONMENT No JS-180/9/2025 Annex 1 c); − Questionnaire for assessing the risk of insurance against civil liability for damage caused by the ingress of dangerous substances into air, water or ground and the costs associated with the removal, cleaning and disposal of any pollutants in Annex 1 d); − List of locations and assets of Annex 2 (a); − Situational maps of the locations of objects at ul. Nowohucka Annex 2 (b); − Situational maps of the locations of objects at ul. Krzemieniecka Annex 2 (c); − Situational maps of the locations of objects at ul. Barska Annex 2 d); − Situational maps of the locations of objects at ul. Blackberry Annex 2 (e); − Situational maps of the locations of objects at ul. The Contracting Authority informs that Miejskie Przedsiębiorstwo Oczyszczania Sp. z o.o. in Krakow is a civil protection entity in accordance with Order No 2016/2025 of the Mayor of the City of Krakow of 26 August 2025 on the designation as civil protection entities of a single-member company of the City of Krakow for the performance of civil protection or civil protection tasks. In light of the above: − The Ordering Party shall make available to the Contractor the contents of Annex II. 1a) – 3) to the FTZ at its written request, after signing a declaration of confidentiality of the data provided. − The Contracting Authority acting in accordance with Art. 2 pts. 2 of the Public Procurement Act shall make available to the Contractor for inspection at the seat of the Contracting Authority documents relating to fire and anti-theft security (formal and legal documents, instructions) after signing a declaration of confidentiality of the data provided. The Contractor is obliged to keep confidential the content of all documents and information obtained in connection with the conduct of these proceedings, in accordance with the principle set out in Article 35 et seq. of the Act of 11 September 2015 on insurance and reinsurance activities (i.e. Journal of Laws 2024, item 838). Local vision (risk illustration): The contracting authority acting in accordance with Art. 2 of the PPL Act requires that the Contractors carry out a local vision (risk vetting) on the premises located: - at ul. Nowohucka 1 and 1 D, - at ul. Krzemieniecka 40; - at ul. Igołomska 17A; to which the subject of this contract relates. The on-site visit will start on 22.10.2025 at 10.00 a.m. 10:00 from the facilities located in Krakow at 1 and 1D Nowohucka Street (collection at the concierge). Next, representatives of the Ordering Party and the Contractor will go to the facilities located at ul. Igołomska 17 A and objects located at 40 Krzemieniecka Street. The Employer's employees will guide the Contractors around the premises covered by the subject of the contract. This local vision is mandatory, i.e. The Contracting Authority shall reject the Contractor's tender on the basis of Art. 1 point 18 of the Public Procurement Act if the Contractor has not carried out the required vision. During the above-mentioned vision, the Ordering Party's representatives will not respond to the Contractors' inquiries. Answers to inquiries will be provided in accordance with Article 284 of the Public Procurement Act. The costs of carrying out the on-site visit (access, possible accommodation) shall be borne by the Contractor. The Contractor is obliged to contact the Ordering Party's representative in writing (at least 2 days in advance) to the following e-mail address: beata.wolk@mpo.krakow.pl from Monday to Friday, from 6 p.m. 800 to an hour. 1400, informing about the desire to participate in the above-mentioned vision. In the content of the application, the Contractor is obliged to provide the full name of the company and the names of the people who will participate in the vision (no more than two people from each Contractor). After the vision has been carried out by the Contractor, the Contracting Authority will provide him with a confirmation of participation in the vision, issued on the basis of the data contained in the application, which the Contractor is obliged to attach to the tender. 4. Deadline for the execution of the contract: Deadline for the execution of the contract: Basic order - from 01. 04. 2026 to 31. 03. 2027 (12 months) Contract covered by the option right referred to in Article 441 of the Public Procurement Act - from 01. 04. 2027 to 31. 03. 2028 (12 months). On 02.10.2026, the Ordering Party will carry out an audit on the performance of the subject of the contract with the Contractor in terms of updating the Ordering Party's insurance needs, assessing the quality of insurance services provided by the Contractor to the Ordering Party, and the result of the above audit will constitute the basis for the Ordering Party to exercise or not to exercise the option right referred to above.
