Description

1. The subject of the contract is the modernisation of the angiography plant of the Provincial Hospital in Bielsko-Biała, including: supply of the angiographic with the additional equipment required (including USG equipment) with the characteristics specified in Annex 1 to the ToR and the necessary adaptation works of the angiography plant to enable the installation and commissioning of the equipment offered
2. Delivery and assembly and commissioning of the subject-matter of the contract, together with unloading and adaptation work, will be carried out at the Contractor’s expense and risk.
3. The goods offered must be new and unused and have up-to-date CE certificates or declarations of conformity in accordance with Article 11 of the Medical Devices Act of 20 May 2010 (Journal of U. of 2010 No 107, item 679) and comply with the limit requirements set out in Annex 1
4. It is the Contractor’s responsibility to provide instructions for use in Polish, together with the equipment.
5. Provision of a guarantee and post-guarantee service in accordance with the requirements set out in Annex 1.
6. Training of the Contracting Authority’s staff in the operation of the equipment offered in accordance with the requirements set out in Annex 1.
