Description

1. The subject of the order is the Sale together with the delivery of the System for testing the release of APIs from drug forms with a fraction collector and a syringe pump for the project entitled: "Citizine in the development path of a new prolonged-release medicinal product in the treatment of nicotinism" for the University of Rzeszów The subject of the contract has not been divided into parts. The contracting authority shall not allow partial tenders to be submitted. Reasons for not dividing the contract into lots: The subject matter of the contract is a functional whole, the division of the contract into lots would risk excessive technical difficulties and excessive costs of performing the contract, moreover, the lack of division is justified by organisational reasons, any division of the contract could seriously jeopardise the proper performance of the contract, and the need to coordinate the activities of the various contractors performing the individual lots could jeopardise the proper and timely performance of the contract. Moreover, failure to divide into lots does not distort competition by restricting the possibility of applying for them. 2. The detailed description and method of performance of the contract includes the description of the subject of the contract (OPZ), constituting Annex 1.1 to the SWZ 3. Contractors may apply for the award of a contract if they fulfil the conditions relating to: (1) Ability to trade: The contracting authority does not impose a condition in this respect. 2) the right to conduct a specific business or professional activity, if it results from separate provisions: The contracting authority does not impose a condition in this respect. 3) economic or financial situation: The contracting authority does not impose a condition in this respect. 4) technical or professional capacity: The contracting authority does not impose a condition in this respect. 4. The documents constituting the tender to be submitted as the content of the tender. The offer is submitted on the Offer Form - in accordance with Appendix No. 1 to the SWZ. Together with the tender, the Contractor is obliged to attach the following annexes: 1) The current statement as at the date of submission of tenders that it is not subject to exclusion and meets the conditions for participation in the procedure. This declaration shall be submitted by the Contractor in the form of the European Single Procurement Document (ESPD), constituting Annex 2 to Commission Implementing Regulation (EU) 2016/7 of 5 January 2016 establishing a standard form for the European Single Procurement Document. The information contained in the ESPD constitutes a preliminary confirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the procedure. The Ordering Party informs that the instructions for filling in the ESPD and the editable version of the ESPD form can be found at: https://www.uzp.gov.pl/e-uslugi/jedz ; https://www.uzp.gov.pl/knowledge-base/public-procurement-regulation/national-law/single-European-document-procurement . In the case of joint procurement by Contractors, the declaration on the single document form shall be submitted by each of the Contractors jointly applying for the contract. This statement is intended to confirm the absence of grounds for exclusion to the extent specified in this CVT. The Contractor, each participant in the Consortium, submits their own Single European Procurement Documents. In the case of a Contractor who relies on the capacities of other entities, the Contractor shall also submit an ESPD for each of those entities confirming that there are no grounds for exclusion against them and confirming that the conditions for participation in the procedure are met, to the extent that they rely on their resources, signed by persons authorised to represent the entity concerned. In Part IV of the ESPD, the Contracting Authority shall request only a general statement of all selection criteria (Section α), without filling in the individual Sections A, B, C and D; Part V (Limitation of the number of eligible candidates) should be left blank. 2) Declaration of the Contractor/Executor jointly applying for the contract regarding the grounds for exclusion from Article 5k of Regulation 833/2014 and Article 7 par. 1 of the Act on Special Arrangements to Prevent the Support of Aggression against Ukraine and to Protect National Security submitted pursuant to Art. 1 of the Public Procurement Act, the model is attached as Annex 5 to the SWZ. 3) A description of the offered assortment prepared (prepared by the Contractor) and signed by the Contractor, including the name of the manufacturer, model, technical parameters, possibly illustrative photos. The description of the subject of the contract offered by the Contractor is not to be supplemented, because it constitutes the content of the offer by specifying the features hidden under the indicated name of a given assortment / device offered by the Contractor.
