Description

The subject of the contract is the purchase, delivery, assembly, installation and commissioning of the angiograph with dedicated equipment and software, as well as training of the Ordering Party's staff. The subject of the contract will be implemented in the following stages: Stage I – delivery and installation of the Equipment and equipment by the Contractor. Stage II – cooperation between the Contractor and the Ordering Party in obtaining all required acceptances, opinions, arrangements and approvals, including in particular those resulting from the provisions of construction law, sanitary law, radiological protection and other relevant regulations. As part of the implementation of Stage II, the Ordering Party expects to obtain a positive decision authorising the use of the Operating Room and the Device, and then after obtaining the appropriate decision, starting the device. Stage III – implementation of specialist training for the indicated staff of the Contracting Authority and performance of obligations resulting from the guarantee granted, warranty for the device. The detailed description of the subject-matter of the contract and the quantities are set out in Annex 1a to the FTZ – description of the subject-matter of the contract for Package 1 and 2 (OPZ), Annex 1b to the FTZ – description of the subject-matter of the contract for Package 3 (OPZ), Annex 2a to the FTZ – summary of technical and functional parameters for Package 1, Annex 2b to the FTZ – summary of technical and functional parameters for Package 2 and Annex 2c to the FTZ – summary of technical and functional parameters for Package 3. Information on the rules for delivery, guarantees and remuneration is contained in Annex 3 to the FTZ – the draft provisions of the public procurement contract.
