Description

1.The subject of the contract is the implementation of the task entitled ‘Supply of medical devices and apparatus under the Grant Agreement No FENX.06.01-IP.03-0074/25-00/305/2026/57 for the Central Clinical Hospital of the Medical University of Łódź at ul. Pomorskie 251" 2. The detailed description and scope of the subject of the contract are set out in Annex 2 and Annex 2A to the Specifications of the Contract Conditions (SWZ). 3.The Ordering Party divided the order into individual parts and allows the possibility of submitting partial offers - Packages: No 1-No 2 described in detail in Annex 2 and Annex 2A 4.The subject of the contract offered must: - meet the quality and technical standards resulting from the function and purpose, - meet the requirements specified by the Ordering Party in the Terms and Conditions of the Order, in particular the conditions specified in the description of the subject of the contract contained in Appendix No. 2 SWZ - Assortment and Price Form and in Appendix 2A SWZ - Technical Parameters. Failure to meet even one of the limit conditions specified in the above-mentioned Annexes will result in the rejection of the offer. - be free from defects in materials, construction, physical and legal - may not be charged with any rights for third parties, The subject of the contract includes the delivery of medical devices and apparatus for the needs of SP ZOZ CSK UM and bringing to the premises of the Ordering Party, installation, configuration, implementation and checking the correct operation of devices. 5.Implementation of the task within: "Project No FENX.06.01-IP.03-0074/25 ‘Development and modernisation of the CKD Specialist Clinic Team at the Central Clinical Hospital of the Medical University of Lodz’ under measure FENX.06.01 Health system, priority FENX.06 Health of the European Funds for Infrastructure, Climate, Environment Programme 2021-2027 for the Central Clinical Hospital of the Medical University of Lodz at ul. Pomorskie 251" 6.The delivered subject of the contract must have instructions for use in Polish (and how much are required), be accompanied by relevant documents referred to in the FTZ.
