1. Introduction:
The service specification defines the outcomes, services and standards that are required to be delivered by the service provider at the Brynhyfryd Integrated Health and Social Care Centre in Builth Wells.
The service provider will be required to put in place the following services:
- Residential Care Services for 28 (possibly 30) residents within the Brynhyfryd Residential Care Home;
- Reablement support and person centred care for 12 residents within the Short-Term Reablement Unit (registered under CSSIW for Residential Care).
- Facilities management for the whole site.
2. Background:
Powys County Council (PCC) and Powys teaching Health Board (PtHB) are working together with local stakeholders to develop a new model of health and social care in Builth Wells. The new model will replace current services that are no longer able to meet current and future needs, challenges and community based demands for individualised health and social care for the local adult population.
From the new centre the following services will be provided:
- Integrated health and social care “core”team comprising district nursing, OT, care management , domiciliary care and reablement expertise;
- Third sector access and advisory service;
- Outpatient and therapy suite for dietician, physiotherapy, OT, paediatric physiotherapy,
- Outpatients suite supporting tele-health and in-reach (from services outside of Powys) consultant-led outpatients;
- Minor surgery suite in support of the local GP practice;
- Three chair general dental service;
- Ambulance service and paramedic base;
- Well-being suite for group based health education, equipment demonstration, therapy, reablement and day support services;
- 12 en-suite units delivering intensive residential intermediate care with in-reaching nursing and reablement services to meet individualised health and social care needs.
By bringing together health with social care and strengthening partnerships with other statutory services and the third sector we will be able to respond to the needs of the local community quickly and flexibly with the aim of trying to help people to stay independent for as long as possible. The new centre will be a base for staff working in the community and help to ensure that local people can be treated and supported at home whenever possible, therefore improving comfort and quality of life.
The service provider commissioned to deliver all or component parts of this specification will be regarded as a key influencer central to this innovative multi agency, multi disciplinary approach in partnership with the Council and PtHB.
