1.The subject of the order is the supply of a slit lamp simulator (front and back section with gonioscopy) – 1 piece. 2.Short description: Advanced training station, integrating a simulation computer, slit lamp, head phantom and a full educational environment for studying the anterior and posterior segment of the eye, funduscopy and gonioscopy. Equipped with SLT laser learning module – selective laser trabeculoplasty. The device is designed for intensive use in training centers, clinics, medical universities and simulation centers. A detailed description of the subject-matter of the contract can be found in Annex 3 to the FTZ-OPZ. 3.CPV code: 38970000-5 4.The detailed scope of performance of the subject-matter of the contract, which is the draft provisions of the public procurement contract, is contained in the model contract, constituting Annexes No 2 to the FTZ. 5.The required deadline for the implementation of the subject of the contract is: up to 3 weeks from the date of conclusion of the contract / signing of the contract. 6. In accordance with Article 257 of the Public Procurement Law, the contracting authority provides for the possibility of cancelling the procedure in question if the funds which the contracting authority intended to allocate to finance all or part of the contract have not been allocated to it. 7. The contracting authority provides for the use of the so-called reverse procedure referred to in Article 139(1) of the Public Procurement Law, i.e. The contracting authority will first examine and evaluate the tenders, and then make a qualitative selection of the Contractor whose tender was the highest rated, in terms of the absence of grounds for exclusion and compliance with the conditions for participation in the procedure. 9. The Contractor shall be bound by the tender from the closing date for the submission of tenders to 05.12.2026r (90 days). 10. The tender must consist of: 10.1.Contractor's tender form – interactive form available on the e-Procurement Platform; 10.2. the statement referred to in section VII point 1 SWZ - JEDZ - an interactive form available on the e-Procurement Platform; The Contractor is obliged to attach to the Offer a statement valid on the day of submission of tenders that it is not subject to exclusion and meets the conditions for participation in the procedure. The Contractor shall submit this declaration in the form of a European Single Procurement Document, hereinafter referred to as the SED, which is Annex 2 to Commission Implementing Regulation (EU) 2016/7 of 5 January 2016 establishing a standard form for the European Single Procurement Document. The information contained in the SEDZ constitutes a preliminary confirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the procedure. 10.3.declaration referred to in section VII point 1a of the FTZ - (respectively) Declarations Annex No 5 to the FTZ; 10.4.(if applicable) the commitment of the provider referred to in section point 6 of point VII of the FTZ; 10.5.(if applicable) the demonstration of the reservation of information constituting a trade secret in accordance with X point 6; 10.6.document(s) from which the right to sign the Offer arises in accordance with section X point 15 of the FTZ; (if applicable) the relevant powers of attorney in accordance with Chapter X point 16 of the FTZ; 10.7.(if applicable) Statement of contractors jointly applying for the contract - in accordance with Article 117(4) of the Public Procurement Act 10.8.PREVIOUS EVIDENCE MEANS confirming that the deliveries offered by the Contractor meet the requirements specified by the Ordering Party, features: description of the offered subject-matter of the contract - technical specification of the offered subject-matter of the contract together with possible Information for Contractors - allowing to determine the conformity of the offered device/set with the requirements of the Ordering Party presented in Annex 3 to the FTZ, respectively - completed Annex 2 to the FTZ. The Contracting Authority considers the means of proof indicated by the Contractor in Annex 3 to the FTZ OPZ col. Parameters offered – confirmation of compliance with the requirements through the parameters offered by the Contractor. In accordance with Article 107 of the Public Procurement Act, if the Contractor has not submitted the evidence in question or the evidence in question is incomplete, the Contracting Authority will call for it to be submitted or supplemented within the prescribed period (current on the date of its submission).
