The subject of the contract is the supply of catheters and insertion sets for electrophysiology and cryoablation along with the lease of cryoconsole and the lease of an electroanatomical 3D cardiac mapping system. The contractor is obliged to perform the contract on the terms and conditions described in the draft contract, constituting Annex 4 to the SWZ and in the detailed description of the subject of the contract, constituting Annex 1 to the SWZ and (in the case of task No 1-15) Annex 1A to the SWZ and (in the case of task No 1-4, 6-8, 10-11, 13-14) Annex 1B to the SWZ. 2.
