Task 1: Deadline for the provision of services: from the date of conclusion – 31/03/2027 The order will be executed by 1 person Place of provision of services: Clinic of Cardiology – Laboratory of Clinical Electrophysiology, Operational Block A detailed schedule for the provision of services covered by the contract will be agreed with the Awarding Party. The maximum number of hours of benefits per month is: 120 The maximum number of treatments per month is: 15 The contract includes: • Performing RF ablation procedures as an operator or first aid; • Performing and interpreting as part of work in the Department of Cardiology diagnostic tests of patients of IPCZD: exercise tests, ECG test, met ECG test. Holter, electrophysiological examination, implantation and control of pacemakers, cardioverters, defibrillators and re-synchronising pacemakers; • Consultation, qualification of patients and other work as part of the tasks performed in the Department of Cardiology IPCZD; • Keeping patients' medical records in accordance with applicable regulations; • RF ablation procedures will be performed on additional dates, other than hourly-based benefits. • The Recipient of the Order undertakes to conduct scientific works, in accordance with the rules in force in the IPCZD, the results of which in the form of publications will be affiliated with the IPCZD. Scientific papers will be submitted to the Department of Science, and scientific publications will be submitted to the IPCZD Scientific Library. The performance of this obligation shall not give rise to any payment to be made by either Party. • During the performance of the above-mentioned services, the Order Recipient may be accompanied by a learner.
