Description

1. Przedmiotem zamówienia jest ZAKUP I DOSTAWA LEKÓW zgodnie z opisem i wymaganiami zawartymi w Załącznikach Nr 2÷2.27 (arkusz excel) do Specyfikacji (w formularzach asortymentowo – cenowych).
2. Dostawa leków odbywać przez okres 12 miesięcy do apteki szpitalnej Zamawiającego.
3. Zamawiający wyraża zgodę na wycenę preparatów zamienne tj. drażetek zamiast tabletek powlekanych i odwrotnie, tabletek i tabletek powlekanych zamiast kapsułek i odwrotnie, tabletek i tabletek powlekanych zamiast drażetek i odwrotnie, kapsułek zamiast drażetek i odwrotnie, tabletek zamiast tabletek powlekanych i odwrotnie, ampułek zamiast fiolek i odwrotnie.
4. . Termin ważności dostarczonego towaru ma wynosić minimum 9 miesięcy od daty każdorazowej, częściowej dostawy. Ewentualne krótsze terminy ważności będą każdorazowo uzgadniane z Zamawiającym.
