PL-Wrocław: health services
The subject matter of the contract is the provision of occupational medical services covering:
1. Medical examinations and the issuing of medical certificates on the basis thereof of the absence or existence of contraindications to the post occupied by:
(1) Preventive testing of police officers,
(2) Inspection tests of police officers,
(3) Periodic surveys and inspections of police staff,
4) Preliminary testing of persons admitted to the police,
5) Surveys of persons admitted for traineeship at the Police,
6) Specialised testing of drivers,
7) Surveys for sanitary and epidemiological purposes with triple swabs for salmonella, schigella and delivery of a health record book,
8) Sanitary/epidemiological renewal of the certificate for sanitary and epidemiological purposes,
9) Research on changes in terms of service/work,
10) Medical examinations following a medical committee’s report on the official’s ability to perform restricted service and issue medical certificates indicating restrictions on the description of the current post of service,
(11) Psychological consultation in urgent and exceptional cases for the explicit referral of the psychologist of the Provincial Police Headquarters in Wrocław.
2. Carrying out preventive vaccination for police officers/staff (including the purchase of vaccines, carrying out a medical qualification test, issuing vaccination books and informing officers/staff and the Contracting Authority of the dates for further doses of the vaccine) against:
(1) hepatitis B,
(2) tick-stick ignition of brain tyres,
(3) to the terror.
3. The official and civil staff employed in:
1. Miliczu District Police Headquarters,
2. Oleśnica District Police Headquarters
3. The District Police Headquarters in Strzelin,
4. Śląskie District Police Headquarters
5. Vołowie District Police Headquarters
The detailed scope of the tests is set out in Annex 1 to the ToR (description of packages), the tender form (description of additional studies) and Annex 3 (description of workstations).
II. The contracting authority shall not allow the submission of partial tenders. A tender covering the whole subject of the contract shall be required.
III. The place where the services are to be provided – the territory of the city of Wrocław.
A single facility shall contain:
(1) establishments in sufficient numbers to cover the number of seconded occupational doctors assigned to carry out the tasks resulting from the performance of the contract,
(2) a treatment room, including the collection of material for laboratory testing,
(3) the ECE workstation.
The services will be provided on working days from Monday to Friday, at least from 8 a.m. to 15 p.m.
