Description

The subject-matter of the contract is the supply of fixtures for Infiniti Vision System apparatus
for Bloku Op. KL. Ophthalmics, the detailed description of which is given in Annexes No 1—
3 to the ToR
The contracting authority shall allow partial tenders to be submitted. Subject
contracts are 3 tasks
The Contractor shall ensure that the offer is in possession of an up-to-date authorisation for
release for free circulation on the Polish market
The validity of the subject-matter of the contract must be at least 24 months.
from the date of delivery to the Contracting Authority’s premises
Each range must be packaged separately
Each individual package must contain a description and instructions for use in
Polish;
Each individual package must bear a durable and legible date
