1. The subject-matter of this tendering procedure are: Supplies of disinfectants and patient hygiene products, hereinafter referred to as ‘goods’, in accordance with the type of assortment and quantities specified in the ‘Assortment and Price Form’, which is Annex 2 to the Medical University of Lodz, and in accordance with the ‘Offer Form’, which is Annex 1 to the Medical University of Lodz, ul. Żeromskiego 113, 90-549 Łódź, Independent Public Health Care Unit of the Norbert Barlicki University Clinical Hospital No 1, Medical University of Łódź, ul. Kopcińskiego 22, 90-153 Łódź and the Independent Public Health Care Department of the Central Clinical Hospital of the Medical University of Łódź, ul. Pomorska 251, 92-213 Łódź. The order is divided into 14 packages. The Assortment and Price Form, in the appropriate columns of the individual packages, indicates the quantities to be ordered by the Independent Public Health Care Unit of the University Clinical Hospital No 2 of the Medical University of Lodz, the Independent Public Health Care Unit of the University Clinical Hospital No 1 of the Norbert Barlicki Medical University of Lodz and the Independent Public Health Care Unit of the Central Clinical Hospital of the Medical University of Lodz.
