Supply of building materials for PPDB Sp. z o.o. as part of the implementation of an investment in SP ZOZ in Sokółka (ZOL and the Paediatric Branch). The contract results from the need to carry out investments obtained through public contracts for PPDB Sp. z o.o. with its registered office in Sokółka, at ul. Track 12. A. Modernization of SPZOZ in Sokółka as part of the task: “High quality of medical services in the Care and Treatment Department in SPZOZ in Sokółka thanks to the modernization of infrastructure and retrofitting with modern equipment”, implemented as part of the National Recovery and Resilience Plan: Component D ‘Efficiency, accessibility and quality of the health system’ Investment D4.1.1 ‘Development of long-term care through modernisation of the infrastructure of medical entities at district level’; B. Modernization and retrofitting of SP ZOZ rooms in Sokółka. Part 1. renovation of rooms in SP ZOZ in Sokółka in order to create a Paediatric Department in the system design and build. 1. The contract covers the supply of building materials for PPDB Sp. z o.o. (new ones) with all the approvals required by law for use in construction (mark ‘B’, ‘CE’ and attestations and certificates indicated in the individual items of the contract). 2. The subject-matter of the contract (each assortment item) must comply with the Do No Significant Harm (DNSH) principle, in accordance with Article 17 of Regulation (EU) 2020/852 of 18 June 2020. This principle applies to the financing of the contract under the National Recovery and Resilience Plan (NRRP). The DNSH principle means that at no stage of the contract performance – from production, through delivery, use, to possible disposal – can there be significant harm to the EU’s environmental objectives, i.e.: 1) Mitigation of climate change, 2) Adaptation to climate change, 3) Sustainable management of water and marine resources, 4) Circular economy (CEE), 5) Pollution prevention and control, 6) Protection of biodiversity and ecosystems." 3. The contract consists of a basic contract and an option contract. Quantitative scope and general requirements are set out in the table annexed to the description of the subject-matter of the contract. 4. Additional requirements for the execution of the order. 4.1. The subject of the order is to be new, free from defects and damage. The Contractor is obliged to deliver the subject of the contract to the Ordering Party within the time limit specified in the contract for the implementation of the subject of the contract and SWZ. 4.2. Deliveries will be made successively as required by the Ordering Party – response time from the date of submission of the demand in accordance with the deadline indicated in the Contractor's offer, not longer than 5 days from the date of notification of the demand. 4.3. When issuing the subject-matter of the contract, the Contractor shall provide the Contracting Authority with the necessary documents related to the subject-matter of the contract. 4.4. A set of documents – constituting an open catalogue in this contract depending on the type of individual elements of the subject of delivery, confirming the release for free circulation of the subject of delivery and its technical parameters, i.e. declarations of performance, CE declarations of conformity, hygienic certificates, safety data sheets, substance safety data sheets, CE (CE marking) or equivalent documents, manufacturers’ guarantee cards for individual elements of the subject of the contract – will be provided by the Contractor to the Contracting Authority before delivery (final acceptance (partial – if applicable, to the extent required). The Ordering Party is entitled to warranty rights in accordance with the provisions of the Civil Code, regardless of the manufacturer's warranty rights. 4.5. The Contractor shall deliver the subject of the contract at his own expense to the place indicated by the Ordering Party. Unloading at the place indicated by the Ordering Party using the Contractor's means of unloading (e.g. HDS, forklift, pallet truck, car elevator). 4.6. Place of delivery of the subject-matter of the contract – Sokółka, ul. Torowa 12 (or gen. Wladyslaw Sikorski 40). 4.7. Receipt of the subject-matter of the contract includes, in particular, checking the compliance of the subject-matter of the contract with the requirements of the description of the subject-matter of the contract and the Contractor's offer. 4.8. From the act of receipt of the subject of the contract, a delivery and acceptance protocol will be drawn up. 5. A detailed description of the subject of the contract is provided in Chapter II. – III.
