Poland-Krakow: Surgical implants
1. The subject-matter of the contract is the supply of bone assembly implants within the cranial, in accordance with the requirements set out in Annex 1 to these specifications.
2. Additional conditions relating to the subject-matter of the contract:
(a) Term of validity of the subject-matter of the contract: may not be less than 12 months from the date of delivery.
(b) The Contractor must provide the Contracting Authority for use free of charge for the duration of the contract:
— 3 sets of instrumentariums for the mini sub-group,
— 3 toolarium kits for the micro subgroup,
— one set of toolariums for the other sub-groups;
(C) The Contractor will be required to complete and maintain, free of charge, the instrumentation provided to the Contracting Authority under the contract in question.
(D) If new products are offered which have not yet been used by the Contracting Authority, the Contractor will be obliged to train the Contracting Authority’s staff at the Contractor’s reference centre (deadline, number of training courses and number of Hospital staff – to be agreed with the Contracting Authority).
3. A detailed description of the subject-matter of the contract is set out in Annex 1 to the ToR.
4. The other terms of the contract are set out in the draft contract, which is attached as Annex 3 to the ToR.
5. The contracting authority allows partial tenders to be submitted, part of which is package I and package II.
