Description

1. The subject-matter of the contract is the supply of ophthalmic products with the lease of a faco-emulsification apparatus and an optical biometer for the L. Rydygier Regional Hospital in Toruń – Ophthalmic Branch, divided into tasks:
— task 1: supply of intraocular lenses, irrigation solution, sterile sets of materials and accessories for cataract operations by means of facoemulsification together with the leasing of a pico-emulsification apparatus with the possibility of front vitrectomy, optical biometric meter,
— task No 2: supply of viscoelasticians,
— task No 3: supply of thin-chamber lenses for trans-sclerical fixation,
— task No 4: delivery of intra-operation knives,
— task No 5: cannula delivery to the front chamber.
2. The product specification for the products to be procured for tasks 1 to 5 are set out in Annexes 2/1 to 2/5 to the ToR, which the Contractor is required to complete and attach to the tender.
