Description

The subject of the order are successive supplies of microtools and medical devices for ophthalmic treatments. The order is divided into 38 packages (parts). Contracts for individual packages will be concluded at the end of the present proceedings for a period of 12 months. A detailed description of the subject of the contract and the Ordering Party's requirements for the performance of the contract shall specify: a) The delivery specifications are set out in Annex 1 to the FTZ (Tender Form) and the detailed characteristics are set out in Annex 1.1 (Price Form) for individual packages, b) Draft provisions of the contract – Annex 2 to the FTZ. All the requirements set out in the documents indicated above are minimum requirements and their fulfilment is mandatory. Failure to meet the above-mentioned minimum requirements will result in the rejection of the offer as inconsistent with the terms of the contract. Each of the contractors may submit a tender for one or any number of selected packages, but within a given package the tender must be complete (i.e. it must cover all items), otherwise it will be rejected. The Ordering Party does not require the inclusion in the offer of packages for which the Contractor does not submit an offer. The contracting authority does not envisage separating individual items from the packages set out in this FTZ during the tender procedure. The Ordering Party does not allow the possibility of submitting tenders for any selected items in a given part. In individual parts, the full range must be priced in the quantities provided by the Ordering Party. The template of Annex 1.1 (‘Price form’) must be completed in its entirety, without any changes to its content – it forms an integral part of the tender – the Contractor’s declaration as to its content, hence the absence of this annex, containing the content in accordance with the template set out in the PZZ, will result in the rejection of the tender (after the Contracting Authority has carried out all the activities provided for in the Public Procurement Act as regards the evaluation of tenders).
