Description

A detailed description of the products is specified in the description of the subject of the contract in the Assortment and Price Form (hereinafter referred to as the "Summary") divided into packages, constituting Annex No. 2 to the SWZ. The Ordering Party requests that the delivered subject of the contract meet the requirements set out in the Act of 7 April 2022 on medical devices (Journal of Laws No. Journal of Laws 2024, item 1620), had the CE marking, met the requirements indicated in other applicable special regulations and standards concerning the subject-matter of the contract and met the minimum requirements set out in the SWZ. The contractor must have documents confirming the above and is obliged to provide them to the contracting authority at any time and at any time. The Ordering Party requests that the delivered product be sterile (if it is described in the Statement), in a sealed factory packaging. The packaging must contain information about the product, at least the name, catalogue number, expiry date, information indicated in the description in the Statement. The Ordering Party requires that the delivered product has an expiry date of at least 6 months from the date of delivery. The Ordering Party provides for periodic deliveries of medical devices, according to the needs of the Ordering Party, on the basis of "Partial Orders". Partial delivery required - up to 3 working days from the date of placing the order. Detailed conditions for the implementation and settlement of the subject of the contract (delivery, lease) are set out in the essential provisions of the contract constituting Annex 8 to the SWZ. Due to the software used by the Ordering Party, any interference with the assortment and price form is prohibited. The Ordering Party requests that the Assortment and Price Forms be submitted together with the offer in the format of .xls, .xlsx or other spreadsheet format. No changes are allowed by adding columns, deleting columns, deleting sheets of individual parts and others.
