Description

1. The subject of the contract are: Successive supply of medicinal products and other materials to the Hospital Pharmacy Department of the A. Wolańczyk District Hospital Sp. z o.o. in Złotoryja. 2. The detailed description of the subject-matter of the contract includes the FTZ, the Price Form – Annex 2 to the FTZ and the model contract – Annex 7 to the FTZ. 3. The contracting authority shall allow partial tenders to be submitted. Number of tasks – 3. Number of tasks for which the Contractor may submit a tender – 3. As an offer correctly submitted, the Ordering Party will consider the offer submitted for all positions in a given task. The Ordering Party does not allow the submission of bids for individual items. The Contractor may submit a tender for all lots: Task No. 1, Task No. 2. 4. Each Contractor may submit only one tender for the entire contract or for a given task(s). If the Contractor submits more than one tender for the entire contract or for a given task/tasks, all tenders of that Contractor shall be rejected by the Contracting Authority. If the Contractor submits an offer for individual items, the Contractor's offer will be rejected by the Contracting Authority. The Ordering Party reserves the right to check the reliability of the parameters provided by the Contractor in all available sources, including by requesting additional explanations. 5. The Contracting Authority acting on the basis of Art. 1 of the Public Procurement Law Act of 11 September 2019 stipulates that it will first examine and evaluate the tenders and then make a qualitative selection of the Contractor whose tender has been the highest rated as regards the absence of grounds for exclusion and compliance with the conditions for participation in the procedure.
