The subject-matter of the contract is: Task 1 - Successive delivery of kits for extracorporeal therapy with the use of a handle and the creation of a deposit. Task 2 - Successive delivery of disposable equipment for gastric varicocele ligation procedures along with the creation of a deposit along with transport and bringing to the place indicated by the Ordering Party. Detailed description of the subject of the contract includes the Price Form - Annex No. 2.1-2.2 to the SWZ.Each Contractor may submit only one tender for the entire contract or a given task / tasks in writing under pain of nullity. If the Contractor submits more than one tender for the entire contract or for a given task/tasks, all tenders of that Contractor will be rejected by the Contracting Authority. The order consists of 2 tasks. The Contracting Authority accepts the possibility of submitting partial tenders for individual tasks in accordance with the Price Form - Annex 2.1 - 2.2 to the SWZ.
