Description

The microscopy systems are designed for various clinical applications. In addition to the central role in ENT surgery, they should also be suitable for plastic and reconstructive surgery procedures. The systems must allow interventions on both ears without having to reposition the systems and with enough degrees of freedom in all axes to reach even in hard-to-reach places (e.g. larynx). Motorized fine positioning must be possible to enable face-to-face operations. The systems shall have motor-driven focusing and zoom, apochromatic optics and maximum and minimum working distances. The Tivato 700 has a technical uniqueness here with a very large working distance range of 200 - 625mm. This is very important in terms of patient safety and surgical results, as it gives surgeons of different body sizes the highest level of ergonomics, which prevents tremors, especially during longer operations. In addition, higher magnifications can be implemented due to the small working distance and work with longer instruments due to the large working distance, which also has a direct influence on the surgical result. Furthermore, the systems should enable an electrical adjustment of the depth of field. One of the three systems must be set up for fluorescence-based video angiography. The systems must be equipped with xenon light sources that have daylight-like properties and at least 300 W of power, as well as a second light source. In case of failure, the second light source is automatically switched to increase patient safety. The lighting must have functions that protect tissues outside the field of view from unnecessary light exposure. The Tivato 700 has the unique selling point in terms of the tripod, as an active vibration damping is installed. This prevents the vibration of the systems, which can lead to the interruption of the operation, especially with strong enlargements (e.g. inner ear), whereby a direct danger to the patient is possible (e.g. with acute bleeding). The tripod must be movable on castors, each with a brake, to ensure mobility and stability. The distance from tripod to lens must be at least 1.6 m. The microscopes must be tilted 125° to the rear and 45° to the left and right for comfortable operation. The systems must have a central user interface (touch screen). The system must have two individually programmable handles. The systems must allow observers in addition to the surgeon (co-observer tube). The systems must be networkable via WLAN, support live video streaming, 4K videos and DICOM, and have compatibility with existing systems of the OR (e.g. Brainlab Curve and Sony IOPS). The microscope systems shall have an automatic balancing function of all relevant system axes, allowing the system to be balanced at the touch of a button and without manual intermediate steps. This feature is another unique feature of the Tivato 700 and has a positive effect on patient safety by reducing the time and errors required. The automatic balancing must work in any position in the tilt and swivel range and even after covering the system, only it may require a push of a button. Automatic balancing of certain configurations must not require additional weights. The lateral tilting of the microscope must not require rebalancing of the system in order not to delay the surgical process and endanger patients, so the co-observer tube must not move when tilting sideways. For improved hygiene and protection against damage, all light guides, video cameras and cables must be fully covered/integrated. The systems must be able to be prepared with suitable sterile sleeves for the operating theatres. Another unique feature of the Tivato 700 is the automatic extraction function of the sterile sleeves. As a result, the sterile casing is very close to the device, which leads to increased patient safety, as the risk of the casing hindering the surgeon or someone getting stuck on the casing is reduced. Additional extendable and foldable tubes must be available to allow different surgeons to work comfortably. Tubes must be available that can be quickly switched between high magnification and maximum overview of the operating area.
