Description

The subject of the contract is the supply of medicinal products, chemotherapy medicines, immunoglobulins, foodstuffs intended for particular nutritional uses, products used in the ketogenic diet, dressings for patients with EB, medicines under the RDTL. The contracting authority shall allow partial tenders to be submitted. The Contracting Authority shall not limit to the Contractors the number of lots for which they may submit a tender. The Contracting Authority does not allow the submission of variants and electronic catalogues. The contracting authority does not provide for the award of contracts referred to in Art. 1 point 8 of the PPL. The detailed description and method of performance of the contract is contained in the assortment and price table constituting Annex 2 to the SWZ and the model contract constituting Annex 4 to the SWZ. The contracting authority shall provide for the right of option in respect of Lot No 15. A detailed description of the application of the option right is contained in Annex 2 and Annex 4 to the SWZ – Part 15. The Ordering Party declares that being a data controller and having regard to the provisions of the Act of October 5, 2018 on the protection of personal data (hereinafter referred to as the GDPR), it has implemented security measures adequate to the risks identified by it and providing a guarantee of proper security of personal data transferred as part of the procedure.
