Description

Kent County Council and the Clinical Commissioning Groups (CCGs) in Kent are looking to commission a Community Navigation service which is designed to help older people and carers make best use of local services and community support to achieve what is most important to them, and so improve their own health, independence and wellbeing.
The contract will commence on 1.4.2019, and will run for a period of 48 months (expiring 31.3.2023) with the option for 2 one year extensions at the sole discretion of the Commissioning Bodies (the Council and/or the CCGs as appropriate) unless terminated in accordance with the published Conditions of Contract.
The contract will be split into both geographic and service lots in order to best meet the differing needs and support requirements of the eligible Kent population.
