Description

Szczegółowy opis przedmiotu zamówienia oraz ilości określono w załączniku nr 2 do SWZ - formularz asortymentowo - cenowy. Informacje w zakresie zasad realizacji dostawy, gwarancji oraz wynagrodzenia zawiera załącznik nr 3 do SWZ – projektowane postanowienia umowy w sprawie zamówienia publicznego na zasadach umowy sukcesywnej. Przez produkty lecznicze, stanowiące przedmiot zamówienia należy rozumieć produkty lecznicze w rozumieniu ustawy Prawo farmaceutyczne z dnia 6 września 2001 roku. Zaoferowane produkty lecznicze muszą być dopuszczone do obrotu na zasadach określonych w art. 3 lub 4 ust. 1 i 2 lub 4a ustawy Prawo Farmaceutyczne. Oferowany przedmiot zamówienia musi być zgodny z aktualnym na dzień składania ofert Obwieszczeniem Ministra Zdrowia w sprawie wykazu refundowanych leków, środków spożywczych specjalnego przeznaczenia żywieniowego oraz wyrobów medycznych.
