The subject of the contract is the provision of medical transport services for the transport of patients from their place of residence to the Clinic for the Treatment of Chronic Wounds at the Ordering Party and return. The service will be carried out by ordinary transport (T) or basic transport (P). The Contractor is obliged to provide patient transport services from Monday to Friday for the entire duration of the contract during the working hours of the Chronic Wound Treatment Clinic. A detailed description of the subject of the contract includes: Appendix No. 1 to the SWZ - Price form and Appendix No. 1 A to the SWZ - Description of the subject of the contract. Detailed conditions and rules for the implementation of the subject of the contract are governed by the provisions of the "Model Agreement" - Appendix No. 5 to the SWZ.
